converting to vertex form

I am in pressing need of direction in finishing a project in converting to vertex form. I need to finish it by next week and am having a real difficult time attempting to work out a several hard problems. I tried some of the web tutorial sites but have not gotten what I needed so far. I would be really thankful if anyone can assist me.
